CORI Form Weekend
After all weekend Masses
Every year we are required by the Archdiocese of Boston to distribute CORI forms to all paid parish and school lay employees, any layperson who draws a check from the parish, school or agency (including 1099 folks) and volunteers 18 years of age and older ministering to children, youth, elderly, and the disabled or serving within various ministries, including but not confined to: Religious education volunteers, pastoral visitation to the sick and homebound, choir members, Eucharistic ministers, ushers, greeters, lectors and nursing home and hospital volunteers. If you belong to any of these groups, you MUST complete and submit a CORI form! IF YOU DO NOT TURN IN A CORI FORM, YOU CANNOT EXERCISE ANY FUNCTIONS IN THE PARISH!
You must return the form with proof of identification (driver’s license, passport or state ID card with photo) to the Gathering Room after all Masses on CORI weekend. Staff members will be available to take and verify your form.
The CORI check is a requirement of the Archdiocese of Boston AND the Commonwealth of Massachusetts!
